Medication Information & Procedures
Medication administration at school:
Medication forms are available at the school and on the Cullman City Schools website. Medication forms must be updated with a new form each school year. All medications must be picked up and signed out by the parent/guardian when no longer needed by child or at the end of the school year. All medications that are not picked up will be discarded at the end of each year.
Prescription Medications:
You must have your child’s doctor fill out a medication authorization form for all medications that are a prescription. The parent/guardian must also sign the medication authorization form. No medications will be given without the completed form. The medication must be brought to the school nurse and signed in by the parent/guardian. The medication must be in the original Prescription container with the Prescription label on the container. Medication should never be placed in a child’s backpack.
Over the counter medications:
The parent/guardian must complete a medication authorization form and sign the form. No medicine will be given without the form being signed and completed. The medication must be brought to the school nurse and signed in by parent/guardian. The medication must be in a new un-opened container. It must be in the original container with the label. Cough drops are medications. Medication should never be placed in a child’s backpack.
